1. Convert document from Word  to Adobe format.  If you don't have this tab in Word, you can print to pdf.  Make sure you save the pdf with a filename that makes sense.  For example "Internship Registration for Carol Burnette.pdf"



  2.  Once it is converted to PDF format, open it up in Adobe Acrobat Proc DC.  On the right panel click "Send for Signature"


  3. Select "Prepare Form" button

  4. Considerations. You will be formatting  the fields and setting the "Participate Role" in all of the text field properties.
    1. The "COURSE TO BE DROPPED" needs to be required field. Assign to Signer 1 *Do not use the "Signer's Email" field. It does not work*
    2.  Make sure the student signature is a signature field type. Assign to Signer 1
    3.   Make sure the start/end date fields are large enough and required.  Assign to signer 1
    4. Make sure the hours and salary fields are required. Assign to signer 1.
    5. ELCM should be set to "Signer 2"
    6. Dept Faculty should be set to Signer 3"
    7. The two Faculty Supervisor fields should be set to "Signer 4"
    8. Dept Chair should be set to "Signer 5"
    9. College Dean should be set to "Signer 6"
    10. "ELCM Final Review" should be set to signature  field type AND "Participate Role" should be "Sender"

  5.  Click "Send for Signature" and then "Ready to Send" after preparing the form.  Enter is all the email addresses in the order you want the document signed.   Click "Next" after entering in the email addresses and message.

    Since you don' t know who the "Faculty Supervisor" is, you will need to enter in the "Dept Faculty Internship Coordinator" email address TWICE in the "To:" field so that after they sign it, they will receive the document a second time.  The "Dept Faculty" email address is Signer 3 and Signer 4. The second time they receive it, they will delegate it to the Faculty Supervisor.

    The Message field should have specific instructions especially for the "Dept Faculty Internship coordinator" to delegate the "Faculty Supervisor" once they sign it and receive the email.
